We are pleased to provide this electronic method for viewing public Life & Health and Property & Casualty product filings submitted to the Arizona Department of Insurance. If you wish, you can also contact our offices directly at (602) 364-2393, or submit a formal Public Records Request https://insurance.az.gov/home/record-requests.
If you do not have access to a computer, you can call 602-364-2393 to schedule an appointment to view filings at the Arizona Department of Insurance. Appointments are available 8:00 AM to 4:00 PM (MST), Monday through Friday.

If you are unable to find information you need via SFA, or if you need more detailed information about a particular filing, you can submit a Public Records Request using the form on this page https://insurance.az.gov/home/record-requests and email it to propcas@azinsurance.gov or lifehealth@azinsurance.cov. (Note: There is no cost for viewing filings via SFA, but there is a per-page charge for records produced by the AZDOI). You can also call our offices directly at 602.364.2393.
